During your first visit, we conduct an initial intake which includes:

| • Taking notes based on patients' verbal history |
| • Reviwing/analyzing results from previous testing done by the patients' physician(s) |
| • Discussing the results of lab findings, reported symptoms and history |
| • Answering any questions the patient may have |
|
